Follow-up obligations

An escalation is only useful if someone follows it up. QiXMind records whether a role marked the follow-up complete, and when.

When a follow-up is created

When a safety gate fires at escalate severity and the session is signed off, QiXMind creates one follow-up obligation per escalated gate. The deadline comes from the profile: 24 hours by default for adult and youth mental health. Your clinical lead should confirm or change this before any real-patient use. The follow-up deadline tracks a recorded obligation. Urgent concerns require action under your service's immediate-response protocol.

Completion records that a role marked the follow-up done. It does not record what was done.

Due, overdue and completed

  • Due: open and inside its deadline.
  • Overdue: the deadline has passed. Overdue is worked out from the deadline whenever the record is read and by a daily sweep, and an alert is recorded once on the Follow-ups page. QiXMind does not yet send email or pager notifications.
  • Completed: marked complete by a role, with the time. A follow-up confirmed after its deadline is marked late.

Confirming a follow-up

Open Follow-ups in QiXMind, choose the role you are confirming as and select Confirm completed. The confirmation is recorded as a review event. Closing a follow-up never edits the original sign-off.

What the receipt shows

Each receipt carries the follow-up state at the moment it was issued: not required, open, overdue with or without an alert, or closed on time or late, with the closing role. Issuing a receipt after the follow-up status changes produces a new receipt that names the one it supersedes.

QiXMind is designed for clinicians and services. It is not designed to diagnose or grade risk, it does not assign a risk level, and it makes no claim about clinical outcomes. Sign-off by a recorded role is required before a receipt is issued.
